Driving Data Collection and HIT for Social Determinants of Health: AMCP Foundation Research Symposium
Assessing social vulnerability within a covered population is a key initial action for managed care when reducing disparities in health outcomes. Payers developing interventions must first resolve challenges in collecting, storing, aggregating, and analyzing data about social determinants of health (SDOH).
Designing coverage and delivering care in response to the social, economic, and environmental factors that influence health is essential to value-based care. Applying insights from AMCP Foundation’s Trends in Health Care, this symposium will surface possible data and research solutions to facilitate interoperable data systems and efficient care coordination by plans of all sizes.

Describe health care information technology (HIT) complexities frequently encountered by payers when assessing patients and populations holistically.

Identify at least one tactic to improve the data available within health care delivery systems.

Characterize a strategy that successfully alleviates SDOH barriers in collaboration with providers and community health partners.
